Terms

The terms ‘I’ and ‘my’ refer to Jennifer Cornish trading as Jennifer Cornish Psychotherapy. The terms ‘you’ and ‘your’ refer to the client.

Commencement date

The therapy will begin at the first session and may be terminated by either party with one week’s notice.

Services

The services will include individual therapy or couples therapy and will include face to face or telephone therapy.

Ethical Standards

I am under an obligation to provide therapy in keeping with the ethical standards laid down by the BABCP (British Association for Behavioural and Cognitive Psychotherapists) and the BACP (British Association for Counselling and Psychotherapy)

Confidentiality

I comply with data protection and therapeutic confidentiality and will only share information on a need to know basis. This confidentiality will only be broken if you are at risk or you poise a risk to someone else. Your consent will always be sought in such situations but occasionally this is not possible.

Data Protection Act 2018

I have a legal duty to keep information about you confidential. I will ask for your GP details but will only share information if there is a risk of harm to you or from you.

Access to non-therapeutic information

I may need to share administrative information for financial or audit purposes. I may also be required to assist in investigations into offences. In these cases, my policy is to be open, honest and transparent with you so that you are fully aware of why I need to share information.

Supervision

I need to attend supervision in accordance with BABCP and BACP guidelines. Your identifying details will remain anonymous unless there is risk to you or from you.

Retention and Storage of Information

This information will be kept manually or digitally for a period of seven years after which it is deleted. If the records are deleted or destroyed due to circumstances beyond my control, I cannot be held responsible.

Influence of alcohol and substances

If I think you are under the influence of alcohol or drugs I will terminate the session and you will pay the full fee. If this continues I will terminate the therapy. 

Contact between sessions

This will be either through e-mail or telephone. If you feel you or another person is at immediate risk you will need to contact your GP or the emergency services. I do not run a crisis or emergency service and am not contactable outside my office hours.

Use of my website

The material on my website or any other online content cannot be relied on as medical, legal, therapeutic or other professional advice. I exclude myself from any loss or liability incurred by using information on my website or from any other online content.

Access of service online

I cannot be held responsible for any interruption or interception of data held online by my website or other online content.

Litigation or legal proceedings

There is a potential conflict of interest in litigation and you must let me know as soon as you can if you are involved in legal proceedings. I cannot usually testify on your behalf in matters relating to divorce, personal injury or custody disputes.

Fees and Payment

I may need to disclose personal data to an insurance company to process the paying of your fees. I review my fees every January and you will be given notice of any changes in the fees. I may charge for any sessions not attended without notice of 24 hours.

Acceptance

By making a payment to Jennifer Cornish Psychotherapy you acknowledge and agree to these terms and conditions and my privacy policy. These terms and conditions are subject to change at any time, and the date of the update will be noted below.
